1ShoppingCart (1SC) opt in form giving error “Your IP address has been blocked”

If you see an error like this when submitting a form integrated using the “Custom Form” integration with 1ShoppingCart:


The fix for this is quite straight forward – you just need to ensure that you have correctly assigned the email or name and email fields from the dropdowns:
